Services

Altamedix ADHC provides a bundled service package pursuant to each participant's individual plan of care, developed by the center's multidisciplinary team. The bundled service package includes the following:

Core services:

• Nursing services: observation and monitoring, medication management, blood pressure monitoring, liaison with other health care providers, assistance with personal care, blood glucose testing, wound care, injections;
• Social Services: counseling, support, information and referrals;
• Therapeutic Activities: arts and crafts, educational programs, movie club, music appreciation, news and discussion, table games, field trips;
• Nutrition: balanced breakfast, lunch, nutritional counseling;

Additional Services (as specified in the individual plan of care):

• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Mental Health services
• Registered Dietitian services
• Transportation to and from the center
• Other services (respite care, spiritual care, hair stylist, field trips to local stores and museums, music programs and concerts)
